Most people with a bad back walk in asking for the firmest bed we make. After more than 40 years in beds, I think that advice causes as much pain as it cures.
The short answer
- Yes, the right mattress can reduce back and shoulder pain. Not by being hard, but by holding you so nothing pushes back.
- Firm is not the same as supportive. Support matches your weight, age and fitness.
- Sore hip or shoulder on your side? You're lying on the mattress, not in it.
- Front sleepers need the opposite of everyone else: a firmer bed.
- Give a new bed 10 to 14 days before you judge it.
- No bed cures a medical condition. The right one stops aggravating it.
Should I Buy a Firm Mattress if I Have a Bad Back?
Quick answer: Not automatically. Firm and supportive are not the same thing. Support holds your body for your weight, age, and fitness level, with no pressure points.
The public have been conned for years with words like 'orthopaedic'. Even now, most manufacturers are making extra-firm versions. That's going backwards, not forwards.
The easiest thing to sell is a hard bed. Feel that, isn't that great? It's not going to improve anybody's sleep.

| A hard bed | A supportive bed | |
|---|---|---|
| When you lie down | You feel pressure straight away | You think: I can relax on this |
| Your hip and shoulder | Pushed back at, crunched up | Allowed to sink in |
| Your waist | Left hanging, unsupported | In contact with the mattress |
| Through the night | You keep turning to relieve it | You stay settled |
The research agrees. A trial published in The Lancet found people with chronic low back pain did better on medium-firm mattresses than on firm ones.
Why Do My Shoulder and Hip Hurt When I Sleep on My Side?
Quick answer: You're lying on top of your mattress instead of in it. Either the springs are too firm for your weight, or there isn't enough filling between you and them.
Push down on a spring, and it gives, but it also pushes back. That's its job. So we put as much natural filling as we can between you and the spring to take that pressure off you. With only half an inch of filling, it all lands on your hip and shoulder.
Three Signs You're on Top of Your Mattress, Not in It
- Your shoulder or hip is sore in the morning. It couldn't sink in, so it took your weight all night.
- There's a gap under your waist when you lie on your side. Your waistline should touch the mattress.
- You keep turning over. Your body moves off the pressure points, which wakes you.
No Hip or Back Pain. I'm now sleeping much better with no hip or back pain at all.

Is My Sleeping Position Making My Back Pain Worse?
Quick answer: It can be, especially if you sleep on your front. Back and side sleepers need a bed that lets them sink in. Stomach sleepers need the opposite.
| How you sleep | What the bed must do | What usually suits |
|---|---|---|
| On your back | Hold you level, no pressure on the lower back | The right tension for your weight |
| On your side | Let your hip and shoulder sink in | Softer spring, plenty of filling |
| On your front | Stop your stomach sinking and arching your back | Firmer |
I'm going through this right now with a young couple. He was on a Sancy medium. He phoned me: 'When I'm lying on my stomach, my back is hurting.'
You never told me you sleep on your stomach.
If your stomach sinks in, your back arches. A stomach sleeper who isn't going to change needs a firm bed, so I'm making him another one.
If you want things to get better, you need to change how you sleep, or change what you're sleeping on.

I know the habit. I slept on my front as a boy in Zimbabwe because I was hot, and I had to break it.
What Mattress Is Best if You Have a Spinal Condition?
Quick answer: Usually one that is firm underneath but soft on top: a good spring system with plenty of natural filling. And if your condition is medical, your doctor comes first.
The 87-Year-Old Who Always Bought Hard Beds
What he came in with. A big, strong man with a misshapen spine who can only sleep on his side. Years of hard beds, then £3,500 spent elsewhere on a mattress too hot to sleep on. He asked if I knew the medical name for his condition. I told him: I know everything about beds. Why would you come to a bed man asking for medical terms?
What the photo showed. I put him on a hard bed and took a picture of his back. Look at your hip. Now look at your shoulder. 'Yes,' he said, 'my shoulder is a bit sore.'
What we changed.
- The bed: far more filling, an elasticated cotton top that gives more than normal ticking, no polyester, no foam.
- The firmness: medium was too soft for him; firm was right. His shoulder could finally go in.
- The pillow: he'd gone from too firm to far too soft. He's coming back with his own, and we'll try three until one works.
Why Honesty Matters More Than the Sale
I told him I'm about his weight and ten years younger. A car crash smashed my neck, and sport gave me a bad back. My bed will never cure it. It must never aggravate it.
That broke down the barriers. Because it made sense to him, he was ready to talk.
How Long Does It Take Your Back to Get Used to a New Mattress?
Quick answer: Sometimes 10 to 14 days. Muscles that worked hard on your old bed can finally relax, and that takes time.
It's like going back to the gym after a long time away.
- Week one: you may ache. Muscles that held you up all night are being used differently.
- Week two: the bed is doing the work, and your body is learning to let it.
- After two weeks: now judge it.
This kind of fitting, done properly, earned Marshall and Stewart the title of Best Bespoke Luxury Bed Manufacturer 2026 from SME News.

Book NowFrequently Asked Questions
Is a firm mattress good for back pain?
Not necessarily. Your back needs support matched to your weight, shape, and sleep style. A bed that's simply hard creates pressure points and keeps you turning.
Will a topper make my mattress softer?
No, and that's the biggest misunderstanding. The spring dictates the firmness. A topper adds filling between you and the spring, so the bed feels more luxurious, not softer.
What's the best mattress for a stomach sleeper with back pain?
A firmer one, so your stomach doesn't sink in and arch your back. Better still, retrain yourself to sleep on your back or side.
Can my partner and I have different firmness on each side?
Yes. We regularly make beds with a different tension on each side.
Not sure which mattress suits your back?
This guide explains what to look for. Only lying on a mattress tells you whether it takes the pressure off your back and shoulders. If you would like to talk it through, the team in East Sheen is happy to help.
